We are thrilled that you have chosen Shenandoah Valley Christian Academy as your child’s school! Before your child can attend school we need the following items completed and returned to us as soon as possible:
- Family Information Form
- Financial Worksheet and Fees
- Completed Health and Wellness Packet
- Completed and Signed Emergency Medical Release
- SVCA Parent and Student Handbook Acknowledgement
- Signed Enrollment Contract
- Extended Care Program (optional)
- Bus Route Agreement (optional)
- Completed Transcript Request Form
- Original Birth Certificate (copy will be made, and original will be returned)
